颅脑损伤后血清垂体前叶激素甲状腺素水平的变化及意义 |

摘 要:目的 研究颅脑损伤后下丘脑、垂体、甲状腺功能改变及意义。 方法 应用磁性酶联免疫定量分析法检测156例急性颅脑损伤患者血清泌乳素(PRL)、促卵泡生成素(FSH)、促甲状腺激素(TSH)、甲状腺素(T3和T4)水平的变化,并与100例正常对照组比较。 结果 颅脑损伤急性期血清PRL、FSH、T4含量显著升高,而T3显著降低;分组资料表明,颅脑损伤愈严重,昏迷愈深,上述指标变化愈明显。部分病例受伤72 h后及2周后复测,上述指标逐渐恢复正常。结论 颅脑损伤急性期内分泌功能有明显异常变化。 关键词:脑损伤; 垂体激素,前叶; 甲状腺激素; 磁性酶联免疫
Changes of blood anterior pituitary hormone and thyroxine after craniocerebral trauma in patients

Abstract:Objective To study the function changes of hypothalamus, pituitarium and thyroxine in the patients after craniocerebral trauma. Methods By using magnetic enzyme-linked immunosorbent assay, we observed the changes of prolactin(PRL),follicle stimulating hormone(FSH),thyroid stimulating hormone(TSH) and thyroxine(T3, T4) in the blood from 156 patients with craniocerebral trauma.Then, these changes were compared with 100 normal control persons. Results In the acute stage of craniocerebral trauma,PRL,FSH and T4 in the blood were significantly increased, while T3 was decreased greatly. The grouped material showed that the severer brain injury was and the deeper coma degree emerged, the more obviously these indexes changed.These data were close to normal after 3 days and 14 days respectively in some patients with trauma. Conclusions The endocrine function changed significantly in the acute stage of the craniocerebral trauma. Key words:Brain injuries; Pituitary hormones, anterior; Thyroid hormones; Mspeia▲
颅脑损伤不仅造成严重的神经系统功能障碍,而且可引发一系列内分泌代谢变化。1996~1997年笔者随机检测156例急性颅脑外伤患者血清泌乳素(PRL)、促卵泡生成素(FSH)、促甲状腺激素(TSH)、甲状腺素(T3和T4)水平的变化,并讨论其临床意义。
资料与方法
1. 临床资料:男132例,女24例;年龄18~55岁,平均36岁。伤情:CT扫描及手术证实,急性硬膜外血肿55例,急性硬膜下血肿40例,对冲性脑挫裂伤伴脑内血肿34例,广泛性脑挫裂伤伴脑内多发小血肿14例,原发性脑干伤3例,外伤性蛛网膜下腔出血9例,开放性颅脑损伤1例。其中69例伤及下丘脑或其邻近部位。156例中,属中型损伤([1] [2] 下一页 上一个医学论文: 颌面部爆炸伤愈合过程中血管相关生长因子变化和作用的实验研究 下一个医学论文: 创伤性脑水肿的在体研究
