异丙酚麻醉诱导对体循环和肺循环的影响 |

摘要 目的:观察异丙酚麻醉诱导后体循环和肺循环的改变,对气管插管应激反应的影响。方法:11例择期神经外科手术患者采用静注2.5mg/kg异丙酚诱导麻醉,诱导前后及气管插管后记录SP、DP、MAP和HR,应用热稀释技术测定心输出量等参数。结果:诱导后SP、DP、MAP明显下降(P<0.05),CO、SV、LVSWI和SVR均有不同程度降低。PSP、PDP、PMP和PCWP无显著性变化,气管插管后SP、DP、MAP明显升高和HR增快(P<0.05)。结论:异丙酚对肺循环的影响小于体循环,对气管插管引起的应激反应抑制不明显。 关键词 二异丙酚 麻醉,全身 血液动力学 应激
Changes of systemic and pulmonary circulatory function during anesthesia induction with propofol Ren Yonggong,Wang Zhengping,Li Shitong,et al .Deptartment of Anesthesiology,Shenzhen Peoples Hospital,Shenzhen 518020 Abstract Objective:To observe the changes of systemic and pulmonary circulation function after anesthesia induction with propofol.The stress response to intubation was also studied.Method:The 11 patients,scheduled for neurosurgery,were induced with propofol.and SP,DP,MAP,and HR were recorded during induction.The thermodilution technique was used to measure CO.Result:After induction,SP,DP and MAP were significantly decreased (P<0.05),CO,SV,LVSWI and SVR decreased at different degree.There were no statisitical significance changes in PSP、PDP、PMP and PCWP.After intubation,SP、DP、MAP and HR were increased significantly (P<0.05).Conclusion:Propofol has less effect on pulmonary circulation than systemic circulation,and the stress response to intubation can not be inhibited by propofol. 异丙酚(Propofol)是一种新型短效静脉麻醉药,麻醉诱导对循环功能的影响已有许多报道,但对肺循环的影响报道较少。本次临床试验旨在对比异丙酚麻醉诱导对体循环和肺循环的影响以及机体对气管插管应激反应的变化。
资料和方法
选择ASAⅠ~Ⅱ级择期神经外科手术患者11例,其中男5例,女6例,年龄26~64岁。所有患者术前心肺功能检查正常,无高血压病及药物过敏史,肝、肾功能均正常。术前半小时肌注苯巴比妥钠0.1g、阿托品0.5mg。病人进入手术室开放静脉输入乳酸林格氏液,在局麻下行桡动脉穿刺置管,以美国太空实验室(Spac-labs 90303B)多功能监测仪直接动脉测压和监测心电图,输入患者身高和体重,以便测出体表面积。1%利多卡因5ml局部浸润,经右颈内静脉穿刺放置F7.0Swan-Ganz导管,当导管尖端到达楔压部位后,让患者安静10分钟测定记录心率(HR)、动脉收缩压(SP)、舒张压(DP)、平均动脉压(MAP)、中心静脉压(CVP),肺动脉收缩压(PSP)、舒张压(PDP)、平均肺动脉压(PMP)和肺小动脉楔压(PCWP)。应用热稀释技术测定记录心输出量(CO)、心脏指数(CI)、搏出量(SV)、周围血管阻力(SVR)、肺血管阻力(PVR)、肺血管[1] [2] 下一页 上一个医学论文: 脑电图双频谱指数预测七氟醚麻醉病人切皮体动反应的临床评价 下一个医学论文: 腹腔热化疗对防治胃肠道癌术后复发的临床意义
