心包内全肺切除术83例报告 |

［摘要］ 目的 探讨心 包内全肺切除术的适应证，手术技术，心包缺损修补和心脏疝预防，术后心律失常的处理等 问题。方法 回顾性分析自1993年1月至1999年3月施行的心包内全 肺切除术83例的病例资料。结果 全部病例均术中修补心包，其中 术中直接间断缝合心包缺损65例，自体壁层胸膜修补心包17例，涤纶片修补1例。术后无心 脏疝发生。术后并发心律失常8例，均发生在术后1周内。结论 术 中探查才能决定是否行心包内全肺切除术；术中妥善修补心包可预防心脏疝；术后1周内心 电监护是必要的。

The report of intrapericardial pneumonectomy (83 cases)

［Abstract］ Objective To discuss the indication and op erating technique of intrapericardial pneumonectomy,the repair of pericardial d efect,prevention of cardiac herniation and the postoperative arrhythmia.Methods We collected 83 patients who underwent intra pericardial pneumonectomy from January 1993 to March 1999.Results All of the perieardial defects were repaired during operation,by direct suture in 65 patients,parietal pleura in 17 patients and dacron patch in 1 pa tient.Cardiac herniation didn t happen after operation in all patients.8 patient s had postoperative arrhythmia within the first week.Conclusions Intrapericardial pneumonectomy was decided in operation;to repair pericardial defect in operation can prevent cardiac herniation;continue EKG was necessary wi thin one week after operation.

我科自1993年1月至1999年3月共施行心包内全肺切除术83例。本文就心 包内全肺切除术的适应证、手术技术和术后严重并发症进行讨论，以进一步提高对心包内全 肺切除术和并发症的防治认识。
临床资料
一、一般资料：全组83例，其中男性70例，女性13例，年龄20～73岁，平均48.76岁。心 包内左全肺切除术52例，心包内右全肺切除术31例。原发肺癌71例，占85.13%，其它病变12 例。
二、术前检查：术前判断性检查包括胸部平片或断层片、胸部CT/MRI、纤维支气管镜3项 。部分病例只进行了其中1项检查，另外部分进行了其中2项或全部3项检查，见表1。
