【摘要】 目的 探讨聚D-L乳酸(PDLLA)可吸收螺钉临床应用的可行性。方法 用PDLLA可吸收螺钉治疗松质骨骨折20例，同时复习30例金属螺钉治疗松质骨骨折的病历，统计术后体温升高天数和升高的最高值、伤口渗液和感染情况，随访术后半年关节功能，两组进行统计学处理。结果 可吸收内固定组体温升高天数的中位数81 h，最高体温中位数为38.3℃，切口无感染和渗液，术后半年关节功能完全恢复正常。两组结果差异无显著性。结论 PDLLA可吸收螺钉是治疗松质骨骨折较理想的内固定物。

Treatment of spongy fractures with absorbable screws of poly-DL-lactic acid

【Abstract】 Objective To investigate the feasibility of clinical application of absorbable screws of poly-DL-lactic acid. Methods 20 cases of spongy fractures were fixed by absorable screws,case histories were reviewed in 30 cases of the fractures treated by metal screws,the days of fever and the highest temperature were recorded,postoperative infections observed,the joint function followed up for 6 months, and the results of two groups were analysed statistically.Results 81 hours were the time of fever in the absorable screws group,the middle numble of the highest temperature was 38.3℃,no infections and permeation were found after 6 months of operation, the joint functions recovered fully,not showing remarkable statistical difference.Conclusion The absorable screw is a good fixator in the treatment of spongy fractures.

传统的各种骨折内固定均需要二次手术取出金属内固定物，这无论在经济上、精神上还是肉体上都给患者带来了一定负担〔1〕。1997年10月以来我院使用中科院成都分院与华西医科大学联合研制的聚D-L乳酸（PDLLA）可吸收拉力螺钉治疗松质骨骨折20例，笔者将其与金属螺钉内固定30例患者的资料进行了对照比较。
1 材料与方法
1.1 病例资料 本组20例，男12例，女8例，年龄16～62岁。骨折均为关节内骨折或关节周围松质骨骨折，其中踝关节骨折脱位10例，髋臼后壁骨折5例，股骨颈骨折术中植骨块3例，骶髂关节脱位术中植骨2例。所有的骨折移位均超过5 mm，并需要切开复位内固定，本组手术指征与传统金属内固定适应证相同。患者平均在伤后2.1天手术(0～20天)，平均住院时间14天。
