单肺通气期间七氟醚对肺内分流的影响 |

摘要 目的:了解七氟醚对缺氧性肺血管收缩(HPV)的抑制程度。方法:选择34例开胸行非肺部手术病人随机分为七氟醚组(12例)、氟烷组(14例)、氯胺酮组(8例)。在双肺通气30分钟、单肺通气2分钟、10分钟、30分钟及60分钟同时采集动脉血和混合静脉血行血气分析,计算分流率。结果:双肺通气30分钟时三组分流率无明显差别。七氟醚、氟烷与氯胺酮相比能明显增加单肺通气期间肺分流率,抑制HPV,PaO2、PaCO2仍在正常范围。结论:七氟醚能增加单肺通气期间肺分流,抑制HPV,抑制程度明显小于氟烷而大于氯胺酮。 关键词 七氟醚 氟烷 氯胺酮 肺分流 单肺通气
The effect of sevoflurane on lung shunt fraction during one-lung ventilation Ma Hong, Wang Junke,Xu Guozhong,et al.Department of Anesthesiology,First Affiliated Hospital,China Medical University,Shenyang 110001 Abstract Objective :To investigate the effect of sevoflurane on lung shunt fraction during one-lung ventilation(OLV).Method:Thirty-four patients undergoing elective open-thoracic nonpulmonary operation were divided into three groups during OLV,group Ⅰ(n=12): anesthetized with sevoflurane;group Ⅱ(n=14) with halothane;groug Ⅲ(n=8) with ketamine.The mix venous and arterial blood samples were collected for blood gases analysis 30 min following two-lung ventilation(TLV), 2 min, 10 min, 30 min and 60 min following OLV and the shunt fraction was calculated. Result: Sevoflurane and halothane caused significant increases of shunt fraction during OLV ,but the degree in group Ⅱ was more than in group Ⅰ. Ketamin did not inhibit hypoxic pulmonary vasoconstriction (HPV).During OLV,PaO2,PaCO2,PvO2 and pH were significantly unchanged.Conclusion: Sevoflurane inhibites HPV,but the inhibition extent is smaller than that of halothane. 吸入麻醉药对缺氧性肺血管收缩反应(HPV)均有不同程度的抑制作用,使单肺通气期间s/t上升,PaO2下降。本文以开胸行非肺部手术的病人进行观察,比较七氟醚、氟烷及氯胺酮对单肺通气期间肺内分流的影响。
临床资料
选择34例ASAⅠ~Ⅱ级择期开胸行非肺部(食管、贲门、纵隔)手术的成年病人,随机分为七氟醚组H(12例)、氟烷组S(14例)、氯胺酮组K(8例)。三组病人年龄、体重、性别及体温无显著差异(P>0.05)。三组病人均于麻醉前40分钟口服安定7.5~10mg,肌注阿托品0.5mg。麻醉诱导以2.5%硫喷妥钠5~6mg/kg、琥珀胆碱1.5mg/kg、芬太尼5μg/kg静注。行双腔管气管插管。做右侧锁骨下静脉穿刺置管(深度17~18cm),桡动脉穿刺置管以备采动脉血进行血气分析。麻醉维持七氟醚组及氟烷组吸入七氟醚及氟烷0.8~1.8MAC,静注阿库氯铵0.2[1] [2] [3] 下一页 上一个医学论文: APTT监测体外循环后肝素中和与反跳 下一个医学论文: 近膝关节骨折并交叉韧带损伤的早期诊断与治疗
