普鲁卡因对上肢短潜伏期体感诱发电位的影响 |

摘要 目的:探讨静滴普鲁卡因对短潜伏期体感诱发电位(SLSEP)的影响。方法:对上肢感觉传导无异常的病人15例,分别观察静脉滴注普鲁卡因前以及滴注1%普鲁卡因20mg.kg-1.h-110分钟、40mg.kg-1.h-15分钟和60mg.kg-1.h-15分钟的上肢SLSEP,比较N14、N20、P23各波的潜伏期,N14~N20波间潜伏期(CCT)以及N20~P23的峰间值。结果:SLSEP各波潜伏期和峰间值在以上三个不同剂量普鲁卡因滴注后无统计学意义上的变化。结论:普鲁卡因不影响SLSEP术中进行SLSEP监测时,仍可使用普鲁卡因静脉麻醉。 关键词 体感诱发电位 普鲁卡因
Effects of Procaine on Median Nerve Shot Lateney Somatosensory Evocked Potential

Abstract Objective:To study the effect of procaiue on median newe short latency somatosensory evoked potential(SLSEP).Methods:The effects of procaine with 3 different dosages on median nerve SLSEP were studied in 15 cases scheduled for elective neurosurgery without any disturbances in the somatosensory pathways.Procaine was given by infusion in dose of 20mg.kg-1.h-1,40mg.kg-1.h-1 or 60mg.kg-1.h-1 respectively.The latency and amplitude of the SEP responses over the second cervical spinous process(N14) and sensory cortex(N20,P23) were recorded before infusion and after 3 different doses.Results:There were no significant changes in latencies of subcortical N14,cortical N20,central conduction time(CCT) N14-N20 and interwave amplitude N20-P23.Conclusions: It suggests that procaine is a suitable anesthetic to be used in SEP monitoring. 短潜伏期体感诱发电位(SLSEP)在神经外科手术中能有效监测神经传导道的完整性,但在应用时易受到药物尤其是麻醉药物的影响。本文观察了不同剂量的普鲁卡因静脉滴注后对上肢SLSEP的影响。
资料与方法
临床资料 15例ASAⅠ~Ⅱ级的脑外科手术病人,其中男6例,女9例。平均年龄43.4±9.7岁。无检测侧的正中神经体感传导路及皮质区功能障碍。入手术室开放静脉,用容量泵分别滴注1%普鲁卡因20mg.kg-1.h-110分钟、40mg.kg-1.h-15分钟、60mg.kg-1.h-15分钟这三个时段上肢SLSEP的波形变化。滴注普鲁卡因时给予病人面罩吸氧,ECG、BP和SpO2监测。实验结束后常规快诱导插管(2.5%硫喷妥钠、芬太尼和琥珀胆碱),术中麻醉维持用复合液(5%葡萄糖溶液500ml中含普鲁卡因5g、琥珀酰胆碱500mg、哌替啶100mg)和芬太尼。 SLSEP的记录 采用美国Cadwell公司生产的Quanfum84型诱发电位仪记录SLSEP。记录电极分别安放于被刺激上肢对侧头皮的C3&pr[1] [2] 下一页 上一个医学论文: 依托咪酯乳剂在老年人诱导麻醉中的脑电双频和循环的变化 下一个医学论文: 咪唑安定和 或氯胺酮全麻诱导的临床效应观察
