【摘要】目的 探讨体内外颈动脉粥样硬化斑块不同病理成分在高分辨率磁共振上的影像学表现。 方法 22例 (24侧血管) 病人颈动脉狭窄,术前1周行高分辨率MRI (应用特制的相控阵线圈) 检查斑块形态并记录影像。术后切除的粥样硬化斑块在体外行MRI检查并记录影像。将体内外斑块MRI影像与病理切片相匹配,分析体内外斑块的纤维组织、脂质核、钙化、出血及血栓的MRI影像学表现。 结果 体内斑块中的钙化、纤维组织、陈旧出血与血栓、脂质成分在T1加权相 (T1W) 上分别为低信号、等信号、等信号、极高信号,在质子加权相 (PDW) 上分别为极低信号、等信号、高信号、高信号,在T2加权相 (T2W) 上分别为低信号、等信号或轻度高信号、高信号、高信号。体外斑块MRI检查结果与体内斑块检查结果相符合。在T1W及PDW上能够很好的分辨钙化及厚的纤维帽。 结论 应用特制的相控阵线圈,MRI可以初步区分斑块成分,评价斑块的稳定性及观察斑块的动态变化。

MRI analysis of human atherosclerotic carotid plaque composition

Abstract: Objective To explore high-resolotion MRI appearance of atherosclerotic carotid plaque components. Methods Twenty-two patients (24 lesions) scheduled for carotid endarterectomy were examined by high-resolution MRI (using a specially designed phased-array coil) 1 week preoperatively, the postoperative specimens were also examined, and the images of the carotid plaque were recorded. The plaques were transversely sectioned and the pathological findings were matched with images of MRI and correlating MR images of the plaque for analyzing the imaging appearance of the fibrous tissue, lipid deposits, calcified plaque and hemorrhage or thrombus. Results In vivo relative signal intensities of calcified plaque, fibrous tissue, hemorrhage (or thrombus) and lipid deposits were low, equal, equal and very high respectively on T1W MR image; very low, equal, high and high respectively on proton density weighted (PDW) MR image; low, equal or slight high, high and high respectively on T2W MR image.
