The Effect of Depression and Negative Attitudes on Glycemic Control in Diabetics Subjects

Abstract:Objective:To investigate the effect of depression and negative attitudes on plasma glucose and complications in diabetic subjects. Methods:A total of 260 adults with diabetes completed a Diabetes Self-rating Questionaire composed of 16 items on negative attitudes to diabetes and depression. The subjects were divided into depression and non-depression groups according to depression scores. Glycemic control was assessed using fasting plasma glucose (FPG) levels and HBA1C testing. Results:The depressive mood scores were significantly correlated with FPG and HBA1C levels. The scores of negative attitude, somatic symptoms and depressive mood were significantly correlated with the number of complications. The FPG level and the number of complications had significant differences in the two groups, while the HBA1C level had no significant difference in the two groups. Conclusions:Bad emotion were significantly correlated with poor glycemic control. Depression was probably one of the factors that caused diabetes-related complications. 糖尿病是内科多发病与常见病,近年来发病率呈不断上升的趋势。有部分糖尿病患者存在情绪问题和负性态度。国外已有学者对糖尿病人的抑郁状况及其影响作了研究,国内也开始关注这方面的问题。如Leedom等在1991年就报告一组伴有并发症的Ⅱ型糖尿病患者,用Beck抑郁量表测评发现74%患者的得分在临床抑郁范围,35%患者的得分在重度抑郁范围[1]。宁布等人报告糖尿病患者的抑郁发生率比一般人群高13~20%,且不良情绪对糖尿病的代谢控制及病情转归有消极的影响[2]。本研究主要是探讨抑郁情绪和负性态度对糖尿病患者血糖控制的影响。
1 对象与方法
1.1 研究对象 1998年4月至1999年4月在湖南医科大学附属第二医院内分泌科住院的糖尿病人,诊断符合1985年世界卫生组织糖尿病研究组工作会议确定的糖尿病诊断标准[3],共260例,均无其它内分泌疾病。其中男性131例,女性129例;年龄22~78岁,平均58±10.92岁;病程1月~24年,平均为5±4.98年。 1.2研究方法 1.2.1 一般情况 包括年龄、性别、病程、教育程度、经济状况、身高、体重等。
