摘 要 目的:探讨雷公藤碱戊的免疫药理作用。方法:采用系统性红斑狼疮(SLE)病人外周血单个核细胞(PBMC)体外培养观察雷公藤碱戊对SLE病人PBMC的B细胞功能的影响及时效关系。结果:雷公藤碱戊能抑制SLE的PBMC自发性增殖反应以及正常人和SLE病人PBMC对SAC刺激后的增殖反应。同时在培养的72 h前加入雷公藤碱戊均能对正常人PBMC在PWM诱导下的IgG产生呈现抑制效应。结论:雷公藤碱戊对B细胞功能的多个环节具有抑制作用,其抑制B细胞产生IgG 的作用环节可能在于抑制了B细胞的活化增殖阶段。

The study of the effect of Wilforidine on SLE patients B cell simmunofunction in vitro

Abstract Objective:The authors attempted to study wilforidine s immunopharmacologic effect on the SLE patients.Methods:Adopted SLE patients peripheral blood mononuclear cells(PBMC)cultured in vitro and investigated the influence of Wilforidine on PBMC s B cell functions and the time-effect correlation of Wilforidine on PBMC s IgG production induced by PWM.Results:Wilforidine not only inhibited the spontaneous proliferation of PBMC s from SLE patients but also inhibited the proliferation of the PBMC induced by SAC.It could inhibit SLE patients PBMC s IgG production induced by PWM. The time-effect results showed that the Wilforidine had obvious inhibition effect on IgG production when added the drug before 72 hour of the culture time in vitro.Conclusion:These data suggested that Wilforidine could inhibited various B cell s function.The main points of inhibition effect on B cell s IgG production may be on the stage of B cells activation and proliferation.

雷公藤治疗自身免疫性疾病已被大家所熟知,它具有明确的免疫抑制功能。雷公藤碱戊(简称碱戊)是其中的一种活性单体。为进一步了解它的免疫药理作用并推测它是否是雷公藤治疗SLE的有效成分之一,本文采用SLE病人外周血单个核细胞(简称PBMC)体外培养观察碱戊对SLE病人的B细胞功能的影响。
1 材料与方法
1.1 研究对象 SLE患者22例,选自本院自身免疫病专科门诊。病例均符合1982年美国风湿病协会(ARA)修正的诊断标准[1],平均年龄为30.0±8.3岁。其中女性21例,正常人对照选自上海医科大学医学检验系员工,以性别、年龄配对。
1.2 药品与试剂
1.2.1 雷公藤碱戊是雷公藤去皮根的醋酸乙酯提取物中经反复硅胶柱层析而得的结晶,经核磁共振及光谱鉴定而得。
