内皮素在急性肝衰竭时肾功能障碍发生中的作用 |

摘 要 目的：探讨内皮素在急性肝衰竭时肾功能障碍发生中的作用。方法：对硫代乙酰胺(TAA)诱导的急性肝衰竭大鼠肝肾功能及内皮素水平进行动态观察。结果：随着肝损伤的加重，肾功能亦进行性恶化(表现为血肌酐升高，肌酐清除率下降，尿量减少，尿钠分泌降低等)，血浆内毒素逐渐升高，均于48h最为严重。伴随上述变化，肾组织内皮素活性渐升高，亦于48h达高峰。血浆内皮素和肌酐清除率及肌酐清除率与肾组织内皮素水平之间均呈显著负相关，而血浆内皮素水平却无显著变化。结论：内皮素在急性肝衰竭时肾功能障碍的发生中起重要作用，但它与内毒素之间似乎具有某种关系。

Effects of endothelin on renal insufficiency in acute hepatic failure

Abstract AIM：To explore the effect of endothelin in renal insufficiency in acute hepatic failure.METHODS:Dynamic changes of endothelin level liver and kidney functions of thioacetamide (TAA)-induced acute hepatic failure in rats was observed.RESULTS：The more serious of liver injury,the worse of kidney dysfunction was (appeared plasma creatinine increased,creatinine clearance rate,urine volume and urinary sodium excretion decreased).Plasma endotoxin level also elevated gradually.At the meantime,endothelin concentration in renal tissue increased progressively,peak value at 48 hours.There was a negative correlation between plasma endotoxin level with Ccr (creatinine clearance)and Ccr with endothelin concentration of renal tissue.However,there was no significant change in plasma endothelin level.CONCLUSION：Endothelin play an important role in renal dysfunction in acute hepatic failure,but there seem to be a complicated correlation between endothelin and endotoxin.

内皮素(endothelin,ET)最初是由Yanagisawa等从猪主动脉内皮细胞培养上清液中分离出的一种活性多肽，由21个氨基酸残基组成，是迄今发现的最强的缩血管物质之一［2］。肝肾综合征(hepatorenal syndrome,HRS)的特征是肾皮质血管强烈收缩而引起的功能性肾功能衰竭，其血管收缩的机制，目前尚未完全清楚。ET的发现给HRS发病机理的研究提供了一条新的线索，日益引起国内外学者的关注。本文通过对急性肝衰竭伴肾功能障碍的动物血浆及肾组织ET水平的动态观察，探讨其在HRS发生中的作用以及与其它相关因素的关系。
材料与方法
实验动物：成年Wistar大鼠，体重200～250g，由本校实验动物中心提供。
