系统家庭治疗技术的临床运用 一 |

【摘 要】 目的：探讨系统家庭治疗技术的临床运用。方法：运用规范的系统家庭治疗技术进行访谈、干预137例家庭，随访90例。用彩色摄像和高灵敏度录音系统记录每次治疗访谈和随访访谈的全部过程，用相应调查表进行反馈。结果：除绘画练习外，绝大多数常用技术均能较好地应用于中国家庭，但受讨论问题与非问题的比例、治疗师是否中立、系统逻辑、关系现实、对治疗性提问内容理解的程度等多因素的影响。结论：系统家庭治疗常用技术适用于中国家庭，本土化后，可逐渐推广前瞻性研究。

A Study of Clinical Application of Systematic Family Therapy Technique

【Abstract】Objective: To explore the clinical application of the techniques of systematic family therapy. Method: A total of 137 families with index patient were interviewed and intervened by systematic family therapy techniques and 90 families were followed up. The process of each therapeutic or follow-up interview was recorded by color video cameras and high sensitive audio recording system. Meanwhile a self-designed scale was used for feedback. Results: Most general techniques except painting exercises were applicable to Chinese families. But they were influenced by such factors as the ratio of discussing problem to no-problem, whether the therapist was neutral, systemic logic, relationship reality, and understanding degree of the therapeutic questioning. Conclusion: The techniques of systematic family therapy are suitable for Chinese family, and can be generalized and applied.

系统家庭治疗是50年代以来发展起来的一种以家庭为单位的心理治疗技术。70年代由美国传入德国，后又由德国人于1988年传入中国。此种治疗以系统论、控制论为理论基础，将人际互动视为与精神病理相关的重要因素，创造了一系列干预技术，用于处理广泛的临床问题。其特点是：心理治疗师处理临床问题的工作对象不是个体，而是最基本的人际系统家庭［1，2］。
我科自1994年4月起开始进行系统家庭治疗的临床研究，取得较好的效果。现报道其治疗技术在首次访谈和随访访谈中的运用情况，以期进一步探讨此技术的价值。
对象与方法
一、样本
样本是索引病人(index patient,IP)及其家庭成员。IP指呈现精神病理症状或心理卫生问题，并因此而将家庭引入治疗者。有的家庭有一个以上的病人；有的成员虽非病人，却是治疗重点［2］。1994年4月至1998年2月，共对137个有不同诊断的索引病人的家庭进行了系统家庭治疗，其中完成90例随访，随访率为65.7%。
