最后筛房的影像解剖学研究 |

【摘要】目的：为临床开展鞍区手术提供一组最后筛房的影像解剖资料。方法：取50具成人头颅标本，行鼻窦冠状位及水平位薄层CT扫描，重点观测后筛窦最后筛房的影像解剖形态及毗邻关系。结果：测得其矢状径位于5.8～21.5mm之间；重直径位于9.4～26mm之间；左右径位于5.9～23.6mm之间。其中，最后筛房大于或等于同侧蝶窦腔的有23%(23侧)。结论：最后筛房与蝶窦、视神经管之间的解剖关系可形成蝶上筛房、蝶旁筛房或Onodi窦。冠状位及水平位鼻窦CT扫描在后筛窦最后筛房的影像解剖研究及临床手术中具有实际的指导作用。

Imaging anatomy study of the ultimate ethmoid sinus

Objective:Provide image anatomy basis of the ultimate ethmoid sinus for the surgical approach via saddle area.Methods:The ethmoid sinuses of 50 adult heads were coronal and thinner axial CT scanned.The ultimate ethmoid sinus was observed and measured on these CT images.Results:The sagittal diameter of ultimate ethmoid was 5.8-21.5 mm,the vertical diameter was 9.4-26.0 mm and the transverse diameter was 5.9-23.6 mm.In 23% specimens,the ultimate ethmoid cell was larger than or equal to the ipsilateral sphenoid sinus.According to the relative location to sphenoid sinus,the ultimate ethmlid sinus cell will form the super sphenoid-ethmoid cell,side sphenlidethmoid cell or Onodi sinus.Conclusion:The coronal and axial CT scanning of the nasal sinuses should bendfit the its clinical operation and image anatomy study.

后筛窦的最后筛房一般只有一个，位于整个筛窦的最后上方，其大小变异很大，由于其气化发育的程度不同，造成后筛窦与周围毗邻解剖结构的关系复杂，尤其是与蝶窦间的毗邻关系。了解其解剖情况，对临床开展蝶鞍区域的手术具有非常重要的临床意义［1，2］。为此，本文对成人头颅进行高分辨CT扫描来观测后筛窦最后筛房的影像解剖，为临床开展鞍区手术提供一组影像解剖资料。
1 材料和方法
1.1 材料 50具(男性29，女性21)经防腐处理过的完整成年头部标本，均无畸形及外伤改变，鼻窦内无肿瘤、新生物及明显的炎症病变。
CT扫描条件：所用CT机为SEIMENS SOMATOM Plus-S型，窗宽2500，窗位100，电压120KV，电流210mA。
1.2 方法 对50具湿性头颅行鼻窦冠状位、水平位及矢状位CT扫描，扫描时间为2秒，层厚2mm，无间隔。水平位采用与听眦线(OM线，即为外耳门上缘外眦连接)水平平[1] [2] 下一页
