新生儿呼吸机相关性肺炎病原菌分析
郭青云 上官明 黄小霞 【摘要】 目的 了解新生儿呼吸机相关性肺炎常见病原菌和相应敏感药物。 方法 对本院近5年49例呼吸机相关性肺炎患儿下呼吸道分泌物进行细菌培养并做药敏试验。 结果 49例呼吸机相关性肺炎新生儿,下呼吸道分泌物细菌培养阳性42例(85.7%),共检出细菌46株。革兰阴性菌37株,以肺炎克雷伯菌、单胞菌、不动杆菌为主;革兰阳性菌7株,葡萄球菌为主;白色念珠菌2株。革兰阴性菌敏感的主要抗生素为:泰能、环丙沙星、氧氟沙星、舒普深、丁胺卡那霉素。葡萄球菌对万古霉素均敏感,泰能亦有较高敏感性。细菌培养阳性率与上机天数成正比,上机2天以上细菌培养阳性率达71.4%;4天以上细菌培养全部阳性。 结论 呼吸机相关性肺炎常见的致病菌以革兰阴性菌为多,对一般抗生素不敏感,泰能、舒普深,氧氟沙星及环丙沙星敏感性较高。上呼吸机时间越长,呼吸道分泌物细菌培养阳性率越高。 【关键词】 新生儿 呼吸机相关性肺炎 细菌培养 病原菌
Analysis of pathogens for neonate ventilator-associated pneumonia

【Abstract】 Objective To investigate the main pathogens of ventilator-associated pneumonia in neonates and sensitivity of drugs.Methods 49neonates with ventilator-associated pneumonia were performed with bacteria cul-ture and drug sensitivity tests for lower respiratory tract secretion.Results Of the49cases,42were positive(85.7%),46bacterica strains have been detected:37Gram negative bacterica strains;7Gram positive bacterica strains;2candida albicans strains.Klebsiella pneumoniae、pseudomonas aeruginosa、staphylococcus were the main pathogens,and the drug resistance was very high.,but they were relatively sensitive to imipenem,quinolone,sulperazone and vancomycin.The bacterica detection rate had relationship to the day of mechanical ventilation,cases with mechanical ventilation for more than two days,the bacterica detection rate was71.4%,for more than four days,the rate was100%.Conclution Neonates with mechanical ventilation for more than two days had high bacterica detection rate,Gram negative bacterica were the main pathogens,they were relatively sensitive to imipenem,quinolone,sulperazone.
