倾斜试验加硝酸异山梨醇酯舌下含化诊断血管迷走性晕厥 |

【摘要】 目的 为了解异丙肾上腺素倾斜试验(ITTT)加硝酸异山梨醇酯舌下含化诊断血管迷走性晕厥的价值。方法 96例血管迷走性晕厥病人及72例无晕厥病史的健康人分成ITTT加服硝酸异山梨醇酯舌下含组(ITTT+ID)和单纯ITTT组。结果 ITTT+ID组敏感性(86.5%与67.5%,P<0.01)明显高于ITTT组,前者诱发晕厥所需时间(4.53±2.86分钟与6.27±4.11分钟,P<0.05)及实验总时程(24.84±5.15分钟与35.70±6.28分钟,P<0.01)明显短于ITTT组。结论 ITTT加硝酸异山梨醇酯舌下含化是一种敏感性高、特异性强、节时的诊断血管迷走性晕厥的一种方法。 【关键词】 晕厥 倾斜台试验 硝酸异山梨醇酯

Department of Cardiology, Daping Hospital, Third Military Medical University, PLA, Chongqing 400042 【Abstract】 Objective To assess the value of sublingual isosorbide dinitrate administration before isoproterenol tilt table test (ITTT) as a simple practical test for diagnosis of neurocardiogenic mediated syncope (NMS). Methods 96 patients with NMS and 72 healthy controls without syncope were divided into two groups: sublingual isosorbide dinirate administration before ITTT (ITTT+ID) and only ITTT. Results The ITTT+ID group has higher sensitivity (86.5% vs 67.5%; P<0.01) than ITTT group in diagnosis of NMS. The time induced syncope (4.53±2.86 min vs 6.27±4.11min; P<0.05) and the duration of the test (24.84±5.15min vs 35.70±6.28min; P<0.01) were shorter in ITTT+ID group than that in ITTT group. Conclusion Sublingual isosorbide dinitrate administration before ITTT is a simple practical test with high sensitivity, powerful specificity and short duration in diagnosis of NMS. 【Key words】 syncope tilt-table test isosorbide dinitrate
近年来,直立倾斜试验(HUT)已成为诊断血管迷走性晕厥的重要手段[1,2],但各家报道的HUT敏感性和特异性相差甚远,如何在提高HUT检查敏感性的同时而仍保持其较好的特异性是一个重要课题。本试验目的在于研究HUT加服硝酸异山梨醇酯舌下含化在血管迷走性晕厥诊断中的价值。
资料与方法
