类风湿关节炎患者T细胞受体BV基因的表达 |

【摘 要】 目的 检测中国人群中类风湿关节炎(RA)患者T细胞TCR BV基因表达的偏移。方法 采用半定量RT-PCR方法，分析6例RA患者的外周血、关节液、滑膜(每例均取三种标本)中新鲜分离的未经IL-2刺激的T细胞TCR BV1～20基因片段的表达。结果 患者外周血与正常人外周血TCR BV基因表达格局相似；患者关节液与滑膜TCR BV基因表达格局相似；而患者关节内(关节液＋滑膜)TCR BV基因表达格局与外周血相比，BV2、BV7、BV11高，BV13.1、BV20低。结论 RA患者关节内T细胞在对TCR BV基因片段的优势表达。

【Abstract】 Objective To detect the biased expression of TCR BV gene in Chinese rheumatoid arthritis (RA) patients.Methods With the semi-quantitative PCR method,the expression of TCR BV 1～20 gene elements of T cells which have not been activated by IL-2 was analysed.These T cells were freshly isolated from peripheral blood,synovial fluid and synovial membrane from six RA patients.Results There was no difference in BV gene expression pattern between PB T cell of RA patients and controls.The expression of TCR BV gene elments was identical in SF and SM T cells;but the BV2,BV7 and BV11 were increased former while the BV13.1 and BV20 were increased latter when the expression pattern was compared between SF＋SM TCR BV gene and PB TCR BV gene of the RA patients.Conclusion There is a prior expression of TCR BV gene elements in the T cells in the arthrosis of RA patients.

编码TCR各链的基因由许多不连续的V、(D)、J、C基因片段组成，在T细胞的发育过程中，通过众多基因片段发生重排等诸多机制，产生极其多样的未成熟TCR谱。未成熟TCR谱经过胸腺的阳性、阴性选择，构成成熟TCR谱。成熟TCR谱在外来抗原的驱动下出现偏移，表现为某些TCR BV基因片段的限制性表达。
类风湿关节炎以外周关节滑膜炎为特征，受累关节滑膜内有大量单个核细胞浸润，其中20%～50%为T细胞。细胞表面的分化抗原表明这些T细胞主要为激活的记忆Th细胞。强烈提示这些T细胞曾接触过抗原。近几十年来，关于RA患者体内T细胞是否限制性表达了某些TCR BV基因片段已有大量的研究报道。最初的方法用Southern印迹和Cβ探针杂交来检测TCRβ链基因的重排，其后运用针对特异性V家族的单克隆抗体的方法，近年应用PCR方法检测TCR BV基因的表达，该方法在基因转录水平进行分析，极度敏感，且可用PCR产物进一步分析TCR CDR3的特性。但是至今未能取得一致的结果，[1] [2] 下一页
