心脏病患者红细胞钙泵 钠泵活性及离子转运与心功能的关系 |

【摘要】 目的 探讨心脏病患者红细胞内Ca2+、K+、Mg2+及红细胞膜上钙泵、钠泵活性与病因、心功能级别、收缩和舒张功能的关系。方法 测定101例不同病因心脏病患者及38例健康人红细胞内Ca2+、K+、Mg2+及红细胞膜上Ca2+-Mg2+ATP酶、Na+-K+ATP酶活性，并用超声心动图测定左室射血分数（LVEF值）及左房充盈分数（Ai/T）、左室前1/3充盈分数（1/3Ti/T）分别表示收缩和舒张功能。结果 心功能Ⅰ、Ⅱ级时，钙泵活性下降，红细胞内Ca2+显著升高，K+、Mg2+明显降低（P均<0.05）；心功能Ⅲ、Ⅳ级时，上述指标变化更明显（P均<0.01），且钠泵活性也显著下降（P<0.05）。上述指标在高血压病+冠心病组、风湿性心脏病组、扩张型心肌病组间差异无显著性。结论 钙泵、钠泵活性和离子转运异常与病因无明确关系，主要与心功能级别以及收缩、舒张功能有关。

Relationship of erythrocytic calcim-, sodium-pump activities, ion transportation and cardiac function in patients of heart diseases

【Abstract】 Objective To investigate the relationship between the intraerythrocytic Ca2+、K+、Mg2+ concentration, Na+ -pump, Ca2+-pump activities of erythrocyte membrane and cardiac function the systolic and diastolic function, and the cause in patients of heart diseases.Methods The intraerythrocytic Ca2+、K+、Mg2+ concentration and Na+-K+ATPase, CA2+-Mg2+ATPase activities of erythrocyte membrane were measured in 101 patients of heart disease and 38 normal controls. Cardiac systolic function (SF) -indicated by LVEF, and diastolic function (DF)-indicated by Ai／T or 1／3Ti／T were observed by echocardiography. Results Ca2+ -pump activities, RBC-Ca2+, RBC-Mg2+ contents were significantly lower while RBC-Ca2+ contents were significantly higher in patients with cardiac function of class Ⅰ, Ⅱ than those of nromal controls (P<0.05). And Na+-pump activity also significantly lower in patients with cardiac function of class Ⅲ, Ⅳ (P<0.05) in addition to further marked changing of above indices (P<0.01). There are no significant differences in above indices among the group of coronary artery disease, essential hypertension, rheumatic heart disease and dilated cardiomyopathy.
