【摘要】 目的 探讨肺癌患者外周血淋巴细胞免疫分型,了解肺癌患者的基线免疫状态。方法 采用流式细胞仪分别检测60例肺癌患者(肺癌组)和20例正常人(对照组)外周血淋巴细胞免疫分型,并对两组进行比较。结果 肺癌患者CD3(+)、CD4(+)、自然杀伤细胞(NK细胞)比例及CD4(+)/CD8(+)比值均低于对照组(P<0.05),CD8(+)细胞比例高于对照组(P<0.05),CD19(+)细胞比例与对照组相比差异无显著性(P>0.05)。结论 肺癌患者免疫功能与正常人群相比存在降低趋势,外周血淋巴细胞免疫分型可作为肺癌患者免疫监测的一种方法。

Clinical significance of changes in peripheral blood lymphocyte
subsets in patients with lung cancer

Abstract:Objective To investigate the baseline immunological status in patients with lung cancer by analyzing the changes in peripheral blood lymphocyte subsets.Methods Lymphocyte subsets in the peripheral blood were detected by flow cytometry in 60 patients with lung cancer and 20 healthy controls.Results The lung cancer group had a lower percentages of CD3(+), CD4(+) lymphocytes, NK cell, and lower CD4(+)/CD8(+) ratio than the control group (P<0.05), and had a higher percentage of CD8(+) cells than the control group (P<0.05). There was no significant difference in the percentages of CD19(+) cells between the lung cancer group and the control group (P>0.05).Conclusion The immunologic functions in patients with lung cancer were generally compromised in comparison with a healthy population. Detection of the variations of lymphocyte subsets in peripheral blood may be used in the monitoring of immune function for patients with lung cancer.

机体免疫功能在抗肿瘤过程中起着重要的作用,不同淋巴细胞免疫分型在机体免疫的调控和抗肿瘤免疫中起重要作用。肺癌的发生、发展与宿主免疫监视功能低下有关。近年来研究发现,肺癌患者免疫功能低下主要表现为T淋巴细胞和NK细胞功能紊乱与缺陷。监测肺癌患者免疫状态有助于合理选择最佳的治疗方法并评价疾病的预后。本研究采用流式细胞仪检测了肺癌患者外周血淋巴细胞免疫分型的变化情况,以初步探讨肺癌患者的免疫功能状态变化,评估病情状况,指导免疫治疗。
1 资料和方法
1.1 研究对象和分组 2006年10月~2007年4月在苏州大学附属第二医院经病理确诊为肺癌的患者60例(肺癌组),其中男性47例,
