低分子肝素钠治疗重症肺心病疗效观察

【摘要】目的: 评价低分子肝素钠对重症肺心病患者的疗效和副作用。方法: 将92例重症肺心病患者随机分为两组,观察组45例,给予低流量持续吸氧、抗感染、祛痰、止咳平喘、改善心肺功能等常规治疗;治疗组47例在常规治疗基础上加用低分子肝素钠5000IU,皮下注射,每日2次,连用7d。结果: 治疗组与观察组治疗后血液流变学及动脉血氧分压比较差异有显著性(P<005)。结论: 低分子肝素钠治疗重症肺心病有显著疗效。

  Curative Effect Observation of the Low Molecular Heparin Sodium Treating the

  Heavy Sickness of Heart Disease from Lung Disorders

  【ABSTRACT】Objective: To investigate the curative effect and side effect of the low molecular heparin sodium in treating critical heart disease from lung disorders. Methods: 92 patients with critical heart disease from lung disorders were randomly divided into two groups.The control group with 45 cases was given continuous low current oxygenbreathing, the antiinfection treatment, phlegm reduction, relieving cough and stopping asthing, improving heart and lungs function and so on. The treatment group with 47 cases was added low molecular heparin sodium 5000IU with hypodermic injection, two times a day for seven days combined with conventional treatment. Results: There are remarkable compared differences on blood rheology and the artery blood pressure between the patients in treatment group and control group, P<005. Conclusion: The low molecular heparin sodium in treating the critical heart disease from lung disorders has the remarkable curative effect.

  损伤裸露的面神经及硬脑膜,造成永久性面瘫和脑脊液耳漏。本组病例中面神经骨管破坏神经裸露3例,1例位于垂直段,2例位于水平段,胆脂瘤侵蚀神经并与之粘连者术前有面瘫;颅中窝脑板骨质破坏硬脑膜暴露4例,1例术前CT提示脑脓肿;迷路炎容易伴有其他中耳炎并发症的原因可能与胆脂瘤破坏广泛侵袭性强有关。

  总之,迷路炎的术前诊断要依靠前庭症状的病史,瘘管试验及颞骨CT检查互相结合,确诊仍需手术探查,术中病变清除彻底是术后恢复的关键所在。
