摘要 目的:研究流式细胞术(FCM)检测血小板相关免疫球蛋白G(PAIgG)的特点及在特发性血小板减少性紫癜(ITP)诊断中的意义。方法:用FCM检测了47例ITP、13例非免疫性血小板减少、10例自身免疫溶血性贫血、18例其它免疫系统疾病患者以及31例健康志愿者的PAIgG,并与ELISA竞争法检测结果比较。结果:FCM 检测ITP患者PAIgG的平均荧光强度和阳性血小板百分率均明显高于正常对照组(P<0.01),11例有峰形异常。ITP患者FCM 检测的阳性率(87.2%)比ELISA法(83.0%)稍高,两种方法的符合率为85.1%。但两种方法都存在特异性不高的问题。结论:FCM检测PAIgG具有快速、简便和灵敏等特点,可作为免疫性血小板减少实验诊断的新方法。Clinical application of flow cytometry for measurement

Abstract Objective:To investigate the characteristics of measurement of platelet-associated IgG (PAIgG) by flow cytometry (FCM) and its significance in the diagnosis of idiopathic thrombocytopenic purpura (ITP).Method:PAIgG was measured by using FCM in 47 patients with ITP, 13 cases with non-immune thrombocytopenia, 10 cases with AIHA, 18 cases with other immune system diseases and 31 healthy volunteers, in comparison with ELISA.Result:With FCM measurement, the level of mean fluorescence intensity (MFI) and percentage positive platelets in patients with ITP was significantly higher than that in control subjects (P<0.01) and an apparent peak shape abnormality was observed in histogram of fluorescence intensity (log) in 11 out of 47 cases of ITP. The positive percentage of PAIgG by FCM in ITP (87.2%) was slightly higher than that by ELISA (83.0%). The rate of accordance of these two methods was 85.1%. However,these two methods showed low specificity.Conclusion:FCM analysis is a rapid, simple and sensitive method for the measurement of PAIgG in patients with immune thrombocytopenia. It may be suitable to clinical diagnostic application.
