BiPAP呼吸机治疗COPD并发 型呼吸衰竭的疗效观察及其对循环呼吸系统的影响 |

摘 要:对慢性阻塞性肺疾患所致呼吸衰竭43例,采用BiPAP呼吸机进行治疗,同时对治疗过程中的呼吸循环系统变化进行观察。结果发现,BiPAP确实有助于呼吸衰竭的纠正和呼吸肌的恢复,从而增加潮气量,降低呼吸频率和耗氧量。对循环系统无明显影响。所以,双水平正气道压力通气不失为治疗COPD合并Ⅱ型呼吸衰竭的一种较好的无创通气模式。 关键词:BiPAP 慢性阻塞性肺疾患 呼吸衰竭
Influences of BiPAP ventilation therapy on respiratory and circulatory system in patients with COPD complicating respiratory failure

Abstract;To examine the influences of BiPAP ventilation therapy on respiratory and circulatory system,43 patients suffering from chronic obstructive pulmonary disease(COPD)with respiratory failure were treated by BiPAP ventilator. The results showed that BiPAP ventilatior was surely helpful to the management of respiratory failure and respiratory muscle fatigue, and therefore increased tidal volume, decreased respiratory rate and oxygen consumption. There was no influence on circulatory system. We conclude that BiPAP ventilation therapy was a good, nontraumatic method for treating COPD with respiratory failure. 慢性阻塞性肺疾患(COPD)所并发的慢性呼吸衰竭,在临床治疗中仅靠低流量吸氧和呼吸兴奋剂治疗往往难以奏效,而给以气管插管行机械通气治疗,不但增加病人痛苦,而且因患者呼吸肌疲劳对呼吸机产生依赖性,从而造成脱机困难。因此,寻找一种简便、易行、病人痛苦少而效果好的纠正COPD并发Ⅱ型呼吸衰竭的方法,成为众多临床工作者研究的课题之一。近年来,无创性人工通气方法越来越多地应用于临床,为纠正由COPD并发的Ⅱ型呼衰提供了一种新方法。我科自1996年以来,对临床上确诊COPD并发的Ⅱ型呼衰的患者,用双水平气道正压力通气(Bi-level positive airway pressure,BiPAP)呼吸机经口鼻面罩气道正压通气治疗,取得了较好的效果,同时对BiPAP方式所引起的循环呼吸系统变化进行了初步观察,现报告如下。
对象与方法
