喉乳头状瘤p53过度表达及与HPV的关系 |

摘 要:为探讨成人型喉乳头状瘤p53基因表达和HPV 16/18感染之间的关系,利用免疫组化及原位杂交技术,对32例成人喉乳头状瘤标本进行了p53和HPV检测。结果显示:p53和HPV 16/18的阳性表达率分别为53.1%和62.5%。在随访中先后有4例发生了恶变。提示p53的过度表达和HPV感染与病变程度和癌变密切相关,二者可以作为一个预测病变转归和预后的重要指标。 关键词:乳头状瘤 喉乳头状瘤病毒 p53基因 杂交
Study of the relationship p53 gene mutation and human papillomavirus infection of adult-onset papillomatosis

Abstract:An immunohichemical analysis and in situ hybridization were done in order to investigate the overexpression of p53 protein and human papillomavirus 16/18 type (HPV 16/18) infection in 32 cases of adult-onset papillomatosis (AOP). The possible relationship between the overexpress of p53 protein and human papillomavirus 16/18 type infection in the lesions were examined. The results showed over expression of p53 gene was found in 53.1% (17/32) samples of laryngeal papilloma, and 62.5% (18/32) samples were positive for HPV 16/18. There were 4 cases having malignant change during the course of follow-up. It suggests over expression of p53 gene and HPV infection are closely related to severity of the lesion and the malignant change. The p53 immunohistochemical determination is well corretated with HPV detection and both could have prognostic value in these tumours. Key words:Papilloma Papilloma virus p53 gene Hybridization▲
成人型喉乳头状瘤是喉部常见的良性肿瘤,已经证实可以发生癌变,癌变率多在10%~30%之间。野生型p53基因具有控制细胞增殖、抑制细胞转化、阻止癌变过程的作用,但它可通过突变、丢失、重排等形成失活,使p53功能丧失。当正常细胞受到病毒或其他致癌物作用时,DNA损伤不能修复,遗传不稳定,突变积累重排加快,促使其发生恶变[1]。
资料与方法
一、资料 选自我院病理科1991年5月~1994年6月收集的成人喉乳头状瘤标本32例。年龄41~80岁。男性14例,女性18例,平均56.4岁。所有标本用10%福尔马林固定后常规石蜡包埋,制成切片,4μm切片用于常规HE染色和免疫组化检测,3μm切片用于原位杂交检测。
