PPH与Milligan Morgan改良手术的比较与选择

　　【摘要】 目的探讨Milligan-Morgan改良手术与吻合器痔切除术(procedure for prolapse and hemorrhoids,PPH)在混合痔治疗中的应用价值。方法 收集50例分别接受了Milligan法和PPH手术进行治疗的Ⅱ～Ⅳ度混合痔患者，对两种术式疗效、手术时间、术后恢复时间、并发症等进行比较分析。结果 与Milligan手术方式相比，PPH手术方式可明显缩短手术时间和住院时间、减少术后肛周疼痛，但费用较高，且对于脱垂的外痔皮赘治疗效果欠佳。结论 PPH与Milligan-Morgan改良手术各有优劣，应当针对不同病人慎重选择、互为补充。

　　【Abstract】 Objective To investigate improved Milligan-Morgan haemorrhoidectomy and stapler hemorrhoidectomy(procedure for prolapse and hemorrhoids,PPH)in the treatment of mixed hemorrhoid.Methods 50 patients with Ⅱ~ Ⅳ mixed hemorrhoid were treated with either Milligan or PPH.The surgical effect,operative time,postoperative recovery time and complications of the two groups were compared.Results The operative time,length of hospitalization and post-operation pain of the PPH group were reduced obviously compared with those of the Milligan group.But PPH was more expensive and unsatisfied for the treatment of prolapsed external hemorrhoid.Conclusion Each of PPH and improved Milligan-Morgan has its own advantages and disadvantages.They should be carefully chosen for different patients and complement each other.

　　长期以来，Milligan-Morgan术(或称外剥内扎术)一直是痔切除的经典方法。该手术的要点是在痔下方做V形切口，沿内括约肌表面向上剥离到痔块的根部，局部缝合结扎，切除痔块组织。该手术方式简单，几经改良，对混合痔的治疗效果良好。PPH手术(procedure for prolapse and hemorrhoids)，又称吻合器痔切除术、痔上黏膜环切术等。在PPH手术开展初期曾一度被认为可以取代传统手术，但随着临床经验的积累和随访期限的延长，人们渐渐发现，PPH还是有一定的弊端，如果单纯采用PPH往往难以达到理想疗效。应该说PPH术式和传统疗法各有优劣，我们通过对接受两种不同手术方法治疗的病人的比较分析，为临床治疗和手术方式的选择提供参考。

　　1 资料与方法

　　1.1 一般资料(表1)

　　我们随机抽取2005年至今接受Milligan-Morgan术的50例混合痔患者为A组;接受PPH手术的50例混合痔患者为B组。A组病人中Ⅱ度痔3例，Ⅲ度痔28例，Ⅳ度痔19例，男22例，女28例，平均年龄46岁;B组病人中Ⅱ度痔5例，Ⅲ度痔29例，Ⅳ度痔16例，男19例，女31例，平均年龄49岁;两组患者均为确诊的混合痔病例，符合2000年中华医学会外科分会肛肠外科学组制定的《痔的诊断暂行标准》[1]，均有药物治疗史，A组有4人有痔硬化治疗史，B组有2人曾接受过痔硬化剂注射治疗。两组资料无统计学意义，有可比性。

　　1.2 方法

　　A组50例病人均接受改良Milligan-Morgan手术。手术采用骶麻或局
