一体化护理模式在围产期保健中的作用研究 |

摘要 为探讨一体化护理模式在围产期保健中的作用，采用产前健康教育，产时呼吸训练、疼痛缓解(音乐、暗示疗法)、美学渗透、Doula式——家庭化分娩等良性护理介入以及协同式护理，产后社区随访，帮助产妇完成“接受→认知→参与→自觉行为”过程。结果增加了产妇的产前认知，缩短了产程，减少了新生儿窒息及产后出血的发生率，提高了产妇的自护能力，伤口愈合良好，子宫复旧、恶露时间缩短，母婴产褥期患病率下降，新生儿生理性体重下降减少，母乳喂养率提高。说明一体化护理模式可提高围产期保健质量。

The Role of Integrated Nursing-care Model in Perinatalhealth Care

Abstract To investigate the role of integrated nursing-care model in perinatalhealth care, the ways of such benign nursing intermediating as prenatalhealth education, breath trainingduring labor, pain relieving (music,hint therapy), aesthetic permeating anddoula shome-like environment for labor as well as cooperative nursing care and postpartum community follow-up were utilized to assist the mothers in completing the course of “receiving→knowing→joining→conscientious action”. The results showed that the prenatal knowledge of the mothers was increased, thedelivery course shortened, both the incidence of birth asphyxia and postpartumhemorrhagedecreased, and the mothers self-care capacity promoted. The time for uterine recuperation and lochia was shortened. The morbidity for both the mothers and infantsduring puerperium was reduced. The infants physiological weight loss was lessened. The breast-feeding rate was increased. It was suggested that the integrated nursing-care model could improve the quality of perinatalhealth care

建立母婴统一管理体系，以保证孕妇及围产儿的安全是我国围产医学追求的目标［1］。近年来，通过创建爱婴医院、建立母婴同室以及整体护理的实施，已使产科工作逐步接近这一目标。但存在将产前、产时、产后护理工作割裂开来的现象。对此，我院于1997年4月开始将健康教育与临床实践的一体化［2］引入产科护理，把临床护理活动向两端延伸，使之贯穿围产期始终，即一体化护理模式，取得较好护理效果。现报告如下。
