COPD合并肺炎患者潮气末二氧化碳分压监测的临床价值 |

摘 要:为探讨潮气末CO2分压(Pet CO2)监测的临床意义,对45例慢性阻塞性肺疾病(COPD)并肺炎的患者Pet CO2与PaCO2、 PaCO2-Pet CO2(Pa-et CO2)与病情严重程度的关系进行比较。结果显示:轻症感染患者中,Pet CO2和PaCO2呈显著相关(r=0.8523,P<0.01) ;而15例并发呼吸衰竭等重症感染患者,两者则无明显相关(r=0.3065,P>0.05);该15例患者的Pa-et CO2则明显高于轻症感染者(P<0.01),表明动态观察COPD合并肺炎患者Pa-et CO2的变化,对预测并发症发生和评估患者预后有一定的实用价值。 关键词:潮气末二氧化碳分压 慢性阻塞性肺疾病 肺炎
Clinical Value of End-tidal PCO2 Monitoring in COPD Patients Accompanied with Pneumonia

Abstract:In order to investigate the clinical implication of end-tidal CO2 pressure (Pet CO2) monitoring, the relationship between Pet CO2 and PaCO2, and Pa CO2-Pet CO2 (Pa-et CO2) and the disease severity was studied in 45 COPD patients accompanied with pneumonia. In the mild infectious patients, Pet CO2 was significantly correlated with Pa CO2 (P<0.01). While in the 15 severe infectious patients associated with pulmonary failure, no correlation between them was found (P>0.05). These 15 patients showed a significant increase of Pa-et CO2 as compared with that in the mild infectious patients (P<0.01). The results indicated that the changes of Pa-et CO2 in COPD patients combined with pneumonia could predict the occurrence of complication and evaluate the prognosis of the patients. 慢性阻塞性肺疾病(COPD)合并肺炎患者病死率较高,早期发现和积极治疗肺部感染及其并发症对于提高患者存活率非常重要。我科自1998年10月至1999年5月对COPD合并肺炎的45例住院患者潮气末二氧化碳分压(Pet CO2)进行了监测,发现其对肺部感染病情评估有一定意义。现报告如下。
1 临床资料
1.1 一般资料 本组45例,其中男30例,女15例,年龄55~75岁,平均(60.6±3.8)岁。45例均有COPD病史,合并肺炎球菌性肺炎15例,金葡球菌性肺炎10例,克雷伯杆菌性肺炎4例,大肠杆菌和变形杆菌混合感染14例,其它2例;并发呼吸衰竭15例,其中呼吸衰竭合并肝损害3例,呼吸衰竭合并肾功能损害者8例、成人呼吸窘迫症候群(ARDS)4例。 1.2 监测方法 以红外线法在40 s内连续测定10次Pet CO2,取其平均数;并用气囊收集3次呼出气,以测定混合呼出气二氧化碳分压(PCO2);在此期间抽取股动脉血作血气分析。比较Pet CO2与PaCO2,动脉和潮气末二氧化碳分压差(Pa-et CO2[1] [2] 下一页 上一个医学论文: 降压药物服药时间的研讨 下一个医学论文: 婴幼儿血压测量部位研讨
