【摘要】 目的 评价以水凝胶弹簧圈为材料的超选择性肾动脉栓塞术在肾损伤中的应用效果。方法 2000~2008年,我科收治肾损伤病人62例,随机分为实验组和对照组,分别采用水凝胶弹簧圈、明胶海绵联合金属弹簧圈为栓塞材料进行超选择性肾动脉栓塞治疗,观察其治疗效果。结果 采用水凝胶弹簧圈的所有病人均取得了良好的临床效果,实验组术后尿液转清时间、手术前后血红蛋白下降水平均低于对照组(t=6.15、6.65,P<0.05),在取得随访的21例病人中未见严重并发症出现。结论 以水凝胶弹簧圈为材料的超选择性肾动脉栓塞术是治疗肾损伤一种安全有效的方法。

ULTRASELECTIVE RENAL ARTERY EMBOLIZATION WITH HYDROGEL SPRING COIL FOR KIDNEY INJURY WANG KUIJUN, XU BIN, YU YIZHI, et al (Department of Urology, the Economic Development Area Hospital, Yantai 264006, China); [ABSTRACT] Objective To evaluate the effect of ultraselective renal artery embolization with hydrogel spring coil in the treatment of renal injury. Methods From year 2000 to 2008, 62 patients with renal injury were treated, in our hospital, by ultraselective renal artery embolization with hydrogel spring coil, spongia gelatinosa or metal coil. Results Those treated with hydrogel spring coil gained a good result. No severe complications were observed. The time of urine became clear was shorter and the decrease of hemoglobin was milder before and afier surgery as compare with the control. Conclusion Ultraselective renal artery embolization with hydrogel spring coil is safe and effective in the treatment of kidney injury.

以水凝胶弹簧圈(HES)为栓塞材料的超选择性肾动脉栓塞术治疗肾损伤出血,是在选择性肾动脉栓塞术的基础上,对技术操作及栓塞材料要求上的进一步提高,以最大限度地减少肾梗死范围,达到止血目的,并避免正常肾实质受损。选择性肾动脉栓塞术,在国外从20世纪70年代开始,至今已经有了很大发展,实现了超选择性肾动脉栓塞技术的进步,同时在栓塞材料上从开始的自体血凝块到明胶海绵的应用,到目前的钢丝环及最先进的弹簧圈。国内从20世纪80年代开始采用该技术,现已达到国际先进水平,目前采用的超选择技术及最先进的栓塞材料,可既快速又准确地使出血肾脏止血,最大限度地减少了肾损伤出血的手术率及切除率,保护了肾脏功能,为肾损伤出血的临床治疗开辟了一条崭新途径。HES是美国Microvention公司研发的新一代弹簧圈,其表面被覆一种遇水膨胀的丙烯酸共聚物——Hydrogel,与普通铂金弹簧圈相比,这种弹簧圈可以更大程度上填塞血管腔,从而提高动脉栓塞成功率,同时降低复发率和再治疗率。我们采用HES治疗肾损伤出血病人28例,取得了满意的效果,现报告如下。
1 资料与方法
