跟骨定量超声在预测原发骨质疏松性脊柱骨折中的应用价值 |

【摘要】 目的:探讨活体跟骨定量超声(QUS)测量在预测原发骨质疏松性脊柱骨折中的应用价值。方法:研究组:16例绝经后妇女伴有骨质疏松性脊柱骨折者;对照组:15例年龄匹配的、无椎体骨折的绝经后妇女。超声测量在跟骨进行,获取超声振幅衰减(BUA)和超声传播速度(SOS)两个参数,并据机内软件自动得出第三个参数刚度Stiffness。结果:QUS测量的三个参数组间差异均有显著性(P<0.05);比较灵敏度与特异度,超声参数以Stiffness最好,其次为BUA。结论:QUS测量在活体上有较高的精确度、灵敏度和特异性,对于原发骨质疏松性脊柱骨折有重要预测价值。 【关键词】 骨质疏松症;超声波;跟骨 Applicational Value of Calcaneus Qus Measurement in Assessing the Osteoporotic Vertebral Deformities SHI Qin,LI Ping (The Central Hospital of Taiyuan, Taiyuan, Shanxi 030009, China;Hengsheng Hospital of Shenzhen in Bao’an District, Shenzhen, Guangdong 518102, China) Abstract: Objective To evaluate the of QUS measurement in assessing the osteoporotic vertebral deformities. Methods The ultrasound parameters broadband ultrasound attenuation (BUA) and speed of sound(SOS) were measured in the calcaneus of 16 postmenopausal study patients with osteoporotic vertebral deformities,15 agematched postmenopausal control subjects,USE the software to get the thirdparameter(Stiffness). Results Significant differences between QUS measurements of the calcaneus were obtained in two groups(P<0.05).Considerinng the sensitivity and the specificity, Stiffness is the best one in thre parameters.The second one is BUA. Conclution Due to good precision,sensitivity and specificity.the QUS measurements in calcaneus in vivo have important value in assessing the osteoporotic vertebral deformities. Key words: Osteoporosis; Ultrasound; Calcaneus 原发骨质疏松症是指单位体积内骨组织含量减少,微观结构退化,致使骨脆性增加以致易于骨折的一种全身性骨骼疾病。骨折好发部位为胸、腰椎体、桡骨远端、股骨上端等,在绝经后妇女以脊柱压缩骨折最多。随我国人口日益老龄化,该病发病率也日益增加,其严重的并发症骨质疏松性骨折给患者带来极大的痛苦,因此,对其早期预防和治疗意义重大。临床上对该病诊断主要依赖于影像学检查,方法多样,有DEXA、QCT、跟骨定量超声(QUS)、MRI等,其中DEXA被WHO定为骨密度测量的金标准。随研究的深入,发现骨质疏松症从病理上不但有骨密度减少,同时伴骨结构及骨弹性的改变,从而提出骨强度或骨质量的概念,骨疏的定义也从单纯的骨密度下降更新为骨强度或骨质量的降低[1],但DEXA、QCT只能测单纯骨密度,对骨结构的研究受到限制,且具辐射性,而QUS、MRI以其无辐射,能综合反应骨强度的特性越来越受到重视,MRI由于设备及价格限制,难以普及,本文仅就QUS的应用价值做一探讨。 1 对象和方法 1.1 测试对象 依据统一标准对31例受试对象分别摄取胸椎、腰椎X线侧位平片,按是否发生[1] [2] [3] 下一页 上一个医学论文: 侧脑室引流治疗重症高血压性脑室内血肿的护理体会 下一个医学论文: 黄斑区盘状水肿及囊样水肿的光学相干断层扫描图像观察
